Projects supported by EMAHSN have been shortlisted for the 2022 HSJ Patient Safety Awards:

  • New technology and processes to help care workers to identify more quickly when a person they are supporting to live in their own homes is becoming unwell
  • A pilot project that looked at innovations to better manage health care for people with learning disabilities living in supported living settings.

EARLY DETECTION OF DETERIORATION IN HOME CARE

We are delighted to have been shortlisted for the Community Care Initiative of the Year at this year’s HSJ Patient Safety Awards.

The joint project between Fosse Healthcare, Nottinghamshire County Council, birdie and EMAHSN has helped to identify health deterioration in people who receive home care services quickly so that they can be treated earlier.

The pilot used digital tools to help bridge the gap between social home care and primary care and support care staff to spot early warning signs that a person was becoming unwell.

Tracey Jackman, Head of Patient Safety at EMAHSN, worked with Fosse Healthcare to evaluate the project. She said: “We are delighted that this project has been shortlisted for an HSJ Patient Safety Award. The findings of the evaluation, which covered one area of Nottinghamshire, show the work could be scalable to benefit many more people who receive care services in their own homes.”

MANAGING DETERIORATION FOR PEOPLE WITH LEARNING DISABILITIES LIVING IN SUPPORTED LIVING SETTINGS

We were also shortlisted for Patient Safety Pilot Project of the Year, for a joint project with West Midlands Academic Health Science Network and the Association of Directors of Adult Social Services. The programme, commissioned by NHS England and NHS Improvement Midlands region, raised awareness of approaches, interventions and methodologies that could improve health outcomes for people with learning disabilities living in supported living settings.

Tracey Jackman, Head of Patient Safety at EMAHSN said: “The pilot was commissioned by NHS England and NHS Improvement in response to the LeDeR (Learning Disability Mortality Review) report. It demonstrated a number of outputs that could be scaled up at a national level, including detection tools that were suitable for use in learning disability settings and ‘keeping well’ tools – both with Easy Read formats. It’s great that the project has been recognised by being shortlisted.”
